 LONG BEACH PREMIER CARD – TERMS & CONDITIONS The issued Long Beach Premier Card is subjected to the following terms & conditions: New Premier Card Membership Sign-Up* A joining fee of $10.00, not inclusive of GST is payable for the application of a new Premier Card. (*LB dollars cannot be redeemed with our temporary Premier Card) Exclusive Privileges: 

•Enjoy 20% Food Rebates on your birthday month (1 Visit, 1 Table).

•Enjoy 20% Food Rebates on your spouse/a loved one’s birthday month (1 Visit, 1 Table).

Validity: 

1 year (The membership expiry date will be shown on the membership card).

Earning of LB$: 

•Members are entitled up to 16% rebates or more on their Total Food Bill from Ala Carte and Set Menu whendining-in.

•Earning of rebates is only eligible for a group of diners not more than 50 people.

•Earning of the rebates is calculated after the redemption of LB$ and do not include tea, towel, peanut, appetisers,beverages, desserts, promotional items, miscellaneous items, service charge, goods & service tax (GST), etc.

•Rebates are not valid in conjunction with any other promotions, vouchers, and discounts.

•Rebates with only be credited with the presentation of the Premier Card upon payment.

•No rebates will be credited if the Premier Card is not presented.

•Rebates can be accumulated and used for future dine-in meal redemptions (LB$1 = SGD$1).

•To qualify for Long Beach Rebates, payment via debit/credit card must be made by the member.

•The name on debit/credit card must be the same as the member’s Premier Card name.

Redemption of LB$: 

•LB$ are not exchangeable for cash or any other goods not limited to voucher.•Redemption of LB$ is to be deducted from the Total Food Bill (excluding taxes)before new rebates are earned.•The credit amount will be subjected to a 10% surcharge for service tax before the offsetting from thefood bill. The balance amount will then be subject to the usual 10% service charge and prevailing Goodsand Service Tax.•Redemption of LB$ is NOT valid 2 weeks before and after the Chinese New Year period, on eve ofpublic holidays, on public holidays, Mother’s Day, Father’s Day, 31st Dec, Festive and SpecialOccasions, including special promotions.•No redemption or crediting of rebates if the physical Premier Card is not presented.Expiry of LB$:•Expiry of LB$ will be in conjunction with the expiry of the one-year membership cycle as stated on thePremier Member Card.•Strictly no extension will be granted. Expired LB$ that are not redeemed will be forfeited.

Mode of Payment: 

The joining fee for LB$ is payable by all major credit cards, cash, or cheques. 

(Cheques can be mailed to our head office.) 

All cheques are to be made payable to: 

LONG BEACH SEAFOOD RESTAURANT PTE LTD. 

Premier Card Processing Time: 

Approximately 2-4 weeks. 

Premier Card Renewal: 

•Free renewal with a min. spends of $1,000.00 within 1 year for Premier 10% members.

•Free renewal with a min. spends of $5,000.00 within 1 year for Premier 12% members.

•Free renewal with a min. spends of $10,000.00 within 1 year for Premier Plus members.

•Free renewal with a min. spends of $15,000.00 within 1 year for Prestige members.

Loss or Damage of Premier Card: 

•It is the sole responsibility of the cardholder to ensure that the right card is returned after each use.

•Long Beach is not liable for the loss of cards and/or rebates accumulated.

•A $10.00 replacement fee is payable for any loss or damage of the Premier Card.

Non-Transferable: 

The Premier Card is non-transferable nor assignable and is ONLY for the personal use and benefit of the person named on the Premier Card. 

Discretion: 

The management of Long Beach Seafood Restaurants reserves its right to amend any privilege or conditionwithout prior notice.

The management of Long Beach Seafood Restaurants is entitled at any time, in its discretion without liability to you, to suspend or terminate your right to use the Premier Card, refuse renewal or replacementof the Premier Card should any of the conditions and privileges be infringed
